A further exciting opportunity has arisen for practiced, expertiseable and highly competent individual/s to join and progress our Advanced Clinical Practitioner (ACP) team. This is a great opportunity to grow and establish advanced expertise within Frailty. Are you a Registered Practitioner in Nursing, Pharmacy, Physiotherapy or Paramedicine and hold current registration with NMC, HCPC or GPhC possessing the significant qualifications stated (or availability to complete them)? If you are expertiseable, adaptable and practiced with an evident passion for clinical practice, you are invited to apply.
The ACP will be responsible for the provision of high quality patient care by assessing, treating and discharging patients with appropriate supervision. The prosperous individual must have practice within the specialty setting to recognise acutely ill patients and the need for urgent treatment from the streaming process and the capability to work autonomously and as part of a Multi-Disciplinary Team. These are additional ACP frailty posts and will include working across both acute and community services to support the 7 day service model. .

To apply for a trainee post, you will need considerable post registration practice, together with evidence of leadership abilities expertise / practice and regular CPD.

If you are considered to be a Trained ACP or equivalent (your current job title may be different if you work in another region), you will be required to demonstrate your level of practice through portfolio and clinical skill assessment.

Trainees will be supported by educational and clinical managers. It is expected that the trainee will complete a full Master's degree in addition to the appropriate clinical competency portfolio dependent upon place of work. candidates who do not hold a first level degree may apply but will need to evidence the capability to study at Masters Level through recent prosperous completion of level 6 or equivalent modules. Training will be required to be completed within a 5 year period unless otherwise agreed.
There is progression to Band 8a on prosperous completion of the MSc and practice.
