Hilton Nursing PartnersĀ are very excited with our "Home to Decide" model in the West Kent area and are looking for Community Care Support Workers to come and work for ourĀ OutstandingĀ CQC rated services.
This is an amazing opportunity to develop yourself further, not only by being a part of this innovated service but also receiving tailored made training by our award-winning Training and Development team.
You will be a part of our "Home to Decide" service working directly with our Registered Nurses in supporting patients who have been identified as needing a residential home and supporting them in having an input in their future, by having them in their own home for a 2 week period, putting support into place and assessments as to whether they are safe to stay longer time.
Hilton understands and promotes that recovery is best in your own home. We are not a timed service as person centred care is at the heart of everything that we do. It is important that all our staff members (known as partners) have an input as working as an overall team is key and helps to develop and improve the patient practice.
We are seeking for enthusiastic and caring individuals, who want to make a different to patients' lives. You don't need to have had practice of health and social care, but you do need to have compassion and a positive attitude.
