As a  Bank Rapid Response Support Workeryou will use your significant previous practice and access to transport to provide an impeccably high standard of care to local service users in their homes and will support with (but not limited to): 
   » Assisting in the provision of a high standard of planned individualized, holistic rehabilitation programmes for patients in the community, under the care of the Rapid Response Team, including recuperative and palliative care.
   » Supporting the management and discharge of patients as part of the rapid response team. Will be required to work in the community in support of this.
   » Being prepared to work over seven days per week in order to meet service needs
   » Supporting general practice in early identification and anticipatory care planning for all patients identified at risk of admission to hospital in order that inappropriate admissions are avoided.
   » Assisting in providing nursing care as delegated by the Lead Therapist/Team Sister within patient's homes. This includes providing short term personal care prior to a care package commencing e.g. washing, dressing, meeting all hygiene needs.
   » Providing clerical and administrative support to the team as delegated by the Lead Therapist/Team Sister
   » Reporting any changes in condition of the patient to the Lead Therapist/Team Sister or in their absence the designated nurse in charge of the caseload.
