A number of exciting opportunities have arisen to develop expertise in differing functions within the Memory and Dementia Service in Shropshire, Telford & Wrekin, including the nationally recognised Hospital Avoidance and to be part of transformation of services being co-produced with people with lived practice, their carers and our system partners.
This innovative service continues to move forward in response to the latest Dementia policy drivers both nationally and locally working closely with our colleagues within the Primary Care Networks delivering a more enhanced service across the County to meet the needs of those younger and older people accessing our services.
We are pursuing highly motivated individuals who must be committed to working holistically to improve the quality of life for people with dementia, their family and carers.
The prosperous candidates will ideally possess excellent communication expertise, determination, drive and an capability to manage their workload in a challenging ever-changing environment. This will work towards providing a quality of care that achieves confidence and effective results to all those who have contact with the service.
The post holder will be required to work 37.5hrs flexibly across 7 days a week including weekends and bank holidays between the hours of 8am-8pm (core hours 9am - 5pm) in order to meet the needs of the service. As this is a community post a full driving licence is essential.
