We are seeking for a great fit and can offer part time, full time, or locum as requested by the prosperous candidate.
This position offers a unique opportunity to make a difference in the lives of those impacted by homelessness, mental illness, substance use, poverty, complex and/or chronic health conditions. The Nurse Practitioner works as a member of an interdisciplinary care team of Physicians and Nurses. The clinical team, in turn, is supported by Social Workers, Physiotherapists, Dietitians, Outreach Workers, Community Developers and support staff. In wrapping services around the client, the Nurse Practitioner will be required to provide clinical support and skills to other program areas within the Health Centre.

REQUIRED QUALIFICATIONS
   » Bachelor or Masters of Science in Nursing
   » Primary Health Care Nurse Practitioner Certificate
   » Registered Nurse, Extended Class in good standing with the Ontario College of Nurses
   » Minimum of 2 years primary care practice, preferably in a community health setting
   » consciousness and knowledge of the social determinants of health
   » Commitment to the underlying principles of harm reduction
   » capability to work with diverse communities with respect, sensitivity, creativity, innovation, cultural knowledge, non-judgement and facilitation skills
PREFERRED SKILLS
   » Excellent written and oral communication skills
   » practice with Electronic Medical Records (Telus Practice Solutions Suite currently in use)
   » capability to maintain complete and accurate records in conformity with the Health Centre’s policies
   » French and other language skills are considered an asset in supporting the communities we serve
